DEPRESSIVE SYMPTOMS (DS) AND CITIES: A SOCIOECONOMIC PERSPECTIVE FOR THE MEXICAN CASE

This article presents an analysis of the proportion of the population that lives in the 20 top cities of Mexico. Population that suffers from moderate to severe Depressive Symptoms (DS) in relation to urban and socioeconomic factors typical of urban territories and comparing them with people living in rural or non-urban environments that suffers DS. To check this, we generated the Complex Index of Socioeconomic and Urban Conditions (CISUC), based on the Mind the GAPS framework, a model that relates the susceptibility or prevention of mental illness in cities based on urban factors, we also used socioeconomic indicators that exist in Mexican cities. For the construction of the ICCSU database, we used data from the National Health and Nutrition Survey in the years 2006, 2012 and 2018-19 and the data of the Mexican Institute for Competitiveness, A.C. and National Institute of Statistics and Geography in the same years. The results obtained from CISUC were reinforced with the use of a panel data model. The findings that we obtained reveal that there is a more important correlation between cities and people who suffer from moderate to severe DS than in rural areas, a condition that intensifies with the socioeconomic conditions of the population, for example, their socioeconomic stratum, their gender, and present urban marginalization. This allows to generate future discussions about other types of diseases such as anxiety, depression, stress, loneliness, and schizophrenia for large population groups. The panel model yields a lower goodness of adjustment, due to the lack of more time points, however, it points out that improvements in socioeconomic and urban conditions slightly reduce depressive symptoms.

Potential impact of prosthetic biomaterials on the periodontium: a comprehensive review.

The success of a prosthetic treatment is closely related to the periodontal health of the individual. The aim of this article was to review and present the importance of prosthetic restorative materials on the condition of the periodontium, the changes that occur in the composition of the subgingival microbiota and the levels of inflammatory markers in gingival crevicular fluid. Articles on the influence of different prosthetic restorative materials on subgingival microbiota and proinflammatory cytokines were searched for using the keywords ¿prosthetic biomaterials¿, ¿fixed prosthesis¿, ¿periodontal health¿, ¿subgingival microbiota¿, ¿periodontal biomarkers¿ and ¿gingival crevicular fluid¿ in PubMed/Medline, Science Direct, Scopus and Google Scholar. The type of material used for prosthesis fabrication together with poor marginal and internal fit can result in changes in the composition of the subgingival microbiota, as well as increased accumulation and retention of dentobacterial plaque, thus favoring the development of periodontal disease and prosthetic treatment failure. Biological markers have helped to understand the inflammatory response of different prosthetic materials on periodontal tissues with the main purpose of improving their clinical application

in patients who need them. Metal-free ceramic prostheses induce a lower inflammatory response regardless of the fabrication method; however, the use of CAD/CAM systems is recommended for their fabrication. In addition, it is presumed that metal-ceramic prostheses cause changes in the composition of the subgingival microbiota producing a more dysbiotic biofilm with a higher prevalence of periodontopathogenic bacteria, which may further favor periodontal deterioration.

Risk, Covid-19 and hospital care in Mexico City: Are we moving toward a new medical practice?

Covid-19 pandemic has entailed new challenges for health care in the Mexican public health sector, producing changes in clinical practices that are now handling patients infected with covid-19 and also outpatient consultations at tertiary-level care hospitals. Some of these challenges are related to the perception of risk held by physicians regarding the possibility of contracting or transmitting covid-19 during their work,and to the management of risk from the standpoint of biomedical organizational culture linked to the material and symbolic conditions of public health services predating the pandemic. We analyze these issues from a anthropological research based on in-depth interviews to physicians that work with covid-19 patients at “Covid-19 hospitals” or “hybrid hospitals” in Mexico City. Covid-19 has arrived in social relations and perceptions of risk in the arena of health care and involves knowing and transforming some structural and symbolic conditions, resignified with the pandemic, for proper medical care.

Las patologías de los recubrimientos en muros y su repercusión en la salud respiratoria de los usuarios

El nivel de calidad del aire interior (CAI) tiene incidencia directa en la salud humana, por eso, es objeto de estudio del diseño bioclimático, toda vez que éste promueve el aprovechamiento de los factores ambientales para la creación y acondicionamiento de espacios en los que los usuarios experimenten confort y bienestar. Además, de impulsar la construcción con sentido común, en donde prevalezca el uso racional de los recursos y una baja dependencia energética. En la presente tesis se pudieron identificar las propiedades de los materiales más comunes que conforman los recubrimientos de muros (cal, cemento, mortero de cemento y yeso) para reconocer los que mejor se adaptan al clima en la Cd. de México, mismos que según se pudo constatar, contribuyen en el control de algunos factores de riesgo (físico: humedad, químico: tóxicos y biológico: microbios) relacionados con una baja calidad del aire al interior de las viviendas. Tal función mitiga o reduce la aparición de algunos de los padecimientos respiratorios más frecuentes en los usuarios como: alergias, asma, rinitis, gripe y sinusitis. Los análisis a los que fueron sometidos los recubrimientos de cuatro materiales ligantes, permitieron evaluar su desempeño ante factores ambientales semi-controlados (temperatura, humedad relativa y concentración de dióxido de carbono) inherentes al funcionamiento del propio espacio interior y al lugar de emplazamiento del mismo. Obviamente, todos los recubrimientos reaccionaron de forma distinta y acorde con las propiedades físicas y químicas de los materiales presentes en cada uno de ellos. Siendo las variaciones más representativas y satisfactorias, las relacionadas con la permeabilidad y el efecto biocida de los recubrimientos hechos con mezcla de cal. Las patologías que mostraron algunos recubrimientos a lo largo del periodo de monitoreo, debido a la insuficiente gestión de la humedad, al alto desarrollo microbiano y a la naturaleza química del material ligante, fueron criptoeflorescencias, moho, bacterias y desprendimiento/diseminación de partículas; mismas que se consideraron definitorias para establecer que al igual que los seres vivos, los materiales de construcción necesitan rangos higrotérmicos específicos, que permitan su óptimo estado de operación. En apego a lo anteriormente citado, es que se considera conveniente concientizar a la población en temas tan importantes para el bienestar social, como la selección de materiales y -consecuente- conservación de los recubrimientos de muros, no sólo para disminuir los costos de ejecución y mantenimiento, sino para aprovechar sus propiedades, ya que tienen una incidencia directa en lo relacionado con el control de las condiciones ambientales básicas, la regulación de la calidad del aire interior y por ende, en la salud de los usuarios. Dicha labor puede iniciar en la etapa formativa de los profesionales de la construcción, con la intención de normalizar el estudio o correcta selección de los sistemas constructivos en función del clima, pero también identificando la función de cada espacio; para garantizar así, que las propuestas de diseño responderán -satisfactoriamente- a los requerimientos de regulación de la humedad, mediante su absorción, transpiración y/o evaporación.

The level of indoor air quality (IAQ) has a direct incidence on the human health, therefore, is the object of study of bioclimatic design, since it promotes the harnessing environmental factors for the creation and conditioning of spaces in which users experience comfort and well-being. In addition, to encourage construction with common sense, where the rational use of resources and low energy dependence prevail. In this thesis it has been identified the properties of the materials that make up the most common wall coverings (hydrated lime, cement, cement mortar and gypsum) for recognizing those that best adapt to the weather in Mexico City, as it could be verified, they contribute to the control of some risk factors (physical:moisture, chemical:toxics and biological:microbes) related with a low indoor air quality at dwellings. This function mitigates or reduces the appearance of some of the most frequent respiratory ailments in users such as: allergies, asthma, rhinitis, flu and sinusitis. The analyses to which the coatings of four binding materials were subjected, allowed to evaluate their performance in the face of semi-controlled environmental factors (temperature, relative humidity, and carbon dioxide concentration) inherent to the operation of the interior space itself and the place of its location. Obviously, all the coatings reacted differently and according with the physical and chemical properties of the materials in each of them. Being the most representative and satisfactory variations, those related to the permeability and biocidal effect of coatings made with hydrated lime mixture. The pathologies that some coatings showed throughout the monitoring period, due to insufficient moisture management, the high microbial development, and the chemical nature of the binding material, were cryptoefflorescences, mold, bacteria and particle shedding/dissemination; they were defining to establish that in similar way to living beings, building materials need specific hygrothermal ranges, to let them works in an optimal state. According with the above, it is considered convenient to raise awareness among the population on issues as important for social welfare, such as the selection of materials and -consequently- conservation of wall coverings, not only to reduce the costs of execution and maintenance, but to take advantage of their properties, since they have a direct incidence in all related with the control of basic environmental conditions, the regulation of indoor air quality and therefore, in users’ health. This work can begin in the formative stage of construction professionals, with the intention of standardizing the study or correct selection of construction systems depending on the weather, but also identifying the function of each space; to guarantee that the design proposals will respond -satisfactorily- to the requirements of humidity regulation, through its absorption, transpiration and/or evaporation.

EVALUACIÓN DE LA ACTIVIDAD Y EXPRESIÓN GÉNICA DE ENZIMAS ANTIOXIDANTES (SOD, GPX, GST, GR y CAT) COMO BIOMARCADORES DE EXPOSICIÓN A FTALATOS EN CULTIVO CELULAR

"El plastificante bis(2-etilhexilo) ftalato (DEHP) altera el equilibrio entre la producción de especies reactivas de oxígeno y las defensas antioxidantes. A pesar de la información disponible sobre los efectos tóxicos del DEHP en organismos modelo, la respuesta celular al DEHP en especies naturalmente adaptadas para tolerar el estrés oxidativo aún no se ha explorado. Los mamíferos marinos están naturalmente adaptados para hacer frente al estrés oxidativo derivado de la isquemia/reperfusión inducida por el buceo. El objetivo de este estudio fue comparar los cambios en los indicadores de estrés oxidativo asociados al DEHP en células de músculo esquelético de humanos (Homo sapiens sapiens) y lobos marinos de California (Zalophus californianus). Se recolectaron muestras de músculo abdominal de mujeres sanas sometidas a cesárea programada, tras obtener su consentimiento informado. Las muestras de músculo de lobo marino se obtuvieron de crías encontradas recientemente muertas en la colonia de Los Islotes, Golfo de California. Las células de músculo esquelético se aislaron y cultivaron en condiciones estándar. Las células se dividieron en dos grupos. Un grupo fue expuesto a DEHP (1 mM) durante 13 días (n=25; exposición), el otro grupo se mantuvo bajo condiciones control, es decir sin DEHP añadido (n=25; control). Se midió la producción de radical superóxido (O2•-), el daño oxidativo, la actividad de enzimas antioxidantes y la expresión génica mediante métodos espectrofotométricos y RT-qPCR. En células expuestas al DEHP la producción de O2•- y la actividad de superóxido dismutasa (SOD) fueron mayores que en las células bajo condiciones control para ambas especies. La actividad de glutatión S-transferasa (GST) y los niveles de carbonilos proteicos (PC) aumentaron significativamente en las células humanas expuestas a DEHP, y no significativamente en las células de lobo marino. Por el contrario, las actividades de glutatión peroxidasa (GPx) y catalasa (CAT) aumentaron significativamente en las células de lobo marino, pero no en las de humano expuestas a DEHP. Se observó una expresión diferencial de los genes implicados en el metabolismo redox entre las células control y expuestas a DEHP y entre especies. Los patrones de expresión de 26 y 19 genes fueron significativamente diferentes en las células humanas y de lobo marino, respectivamente, después de la exposición a DEHP. En las células humanas, la expresión de la GST1 microsomal y la GST (κ, μ, θ, ω y ᴢ) fue mayor,

"The plasticizer bis (2-ethylhexyl) phthalate (DEHP) dysregulates the balance between reactive oxygen species production and antioxidant defenses. Despite the information available on DEHP’s hazardous effects in model species, the cellular response to DEHP in organisms naturally adapted to tolerate oxidative stress remains unexplored. Marine mammals are naturally adapted to cope with oxidative stress derived from diving-induced ischemia/reperfusion. The objective of this study was to compare changes in oxidative stress indicators induced by DEHP in human (Homo sapiens sapiens) and California sea lion (Zalophus californianus) skeletal muscle cells. Abdominal muscle samples were collected from healthy women undergoing planned cesarean surgery at full term after obtaining informed consent. Sea lion muscle samples were obtained from recently deceased pups at Los Islotes rookery, Gulf of California. Skeletal muscle cells were isolated and cultured under standard conditions. Cells were divided into two groups. One group was exposed to 1 mM DEHP for 13 days (n=25; treatment), and the other served as a control (n=25; no DEHP). Superoxide radical (O2•-) production, oxidative damage, antioxidant enzyme activities and gene expression were measured using spectrophotometric methods and RT-qPCR. DEHP exposure increased O2•- production and superoxide dismutase (SOD) activity in both species. While the activity of glutathione S-transferase (GST) and protein carbonyls (PC) levels significantly increased in human cells, these indicators showed a non-significant elevation in sea lion cells. In contrast, glutathione peroxidase (GPx) and catalase (CAT) activities increased significantly in sea lion but not in human cultures exposed to DEHP. Genes involved in redox metabolism showed differential expression between treatments and species. DEHP exposure led to widespread modifications in gene expression patterns, with 26 and 19 differentially expressed genes in human and sea lion cells, respectively. In human cells, DEHP increased GST1 and GST (κ, μ, θ, ω, and ᴢ), while suppressing 8-oxoguanine DNA glycosylase (OGG1), CAT, GR, and nuclear factor erythroid 2-related factor 2 (NRF2) expression, suggesting increased oxidative stress and phase two detoxification processes. In contrast, DEHP increased OGG1, NRF2, GPx2 and SOD3 expression, suggesting that DEHP activates antioxidant defenses in sea lion cells, potentially contributing to maintain redox homeostasis and avoid oxidative...

Propuesta de mejora para la integración del equipo de salud en el código azul en áreas críticas adulto

La presente investigación se realizará en un hospital de segundo nivel de atención de la ciudad de Cuernavaca, el código azul es un sistema de alarma que existe en la mayoría de los hospitales, está conformado por un equipo multidisciplinario de salud que se encuentra capacitado y es convocado de manera inmediata con funciones previamente asignadas para poder intervenir de forma oportuna, eficaz y con calidad a pacientes en paro cardiopulmonar. Este trabajo de investigación propone la integración del equipo de salud en código azul.

Se menciona como fue la historia de la formación de los grupos de reanimación cardiopulmonar y la organización del código azul; fue el primer modelo de estos sistemas y ha demostrado un impacto positivo en la morbimortalidad de los pacientes, la conexión de la teórica Patricia Benner de principiante a experto, se describe como se vive un código azul en el área hospitalaria, la necesidad de estructurar tarjetas que identifique que rol tiene cada uno de los integrantes del código azul, el personal de salud tiene que tener capacidad de reconocer la emergencia del evento, realizando oportunamente las intervenciones del código azul. Metodología adecuada para esta investigación: se decidió un estudio de tipo cuantitativo, longitudinal, observacional, cuasiexperimental y de intervención educativa, será dirigido a todo profesional de salud; área médica, enfermería, camilleros, residentes, pasantes y estudiantes de ambas profesiones de los diferentes turnos que laboran en las áreas críticas del hospital. Se pretende aplicar dos instrumentos de evaluación diagnóstica y de habilidades previo a la capacitación curso- taller con la finalidad de fortalecer los conocimientos y habilidades y posteriormente aplicar los mismos instrumentos de evaluación final. Se implementará un pizarrón en cada servicio, donde describa el rol de la actividad que tendrá cada integrante que conforme el equipo de código azul, así mismo se proporcionará el manual de procedimientos de código azul, guía de práctica clínica impresa que se ubicaran en un lugar estratégico para su consulta y gafetes que describa la función que le corresponde a cada uno de los miembros.

This research will be carried out in a second level care hospital in the city of Cuernavaca. The code blue is an alarm system that exists in most hospitals; it is made up of a multidisciplinary health team that is trained and immediately summoned with previously assigned functions in order to intervene in a timely, effective and quality manner in patients in cardiopulmonary arrest. This research work proposes the integration of the health team in code blue.

It is intended to apply two diagnostic and skills evaluation instruments prior to the training course-workshop in order to strengthen knowledge and skills and then apply the same instruments for the final evaluation. A blackboard will be implemented in each service, describing the role of the activity that each member of the code blue team will have, as well as the code blue procedure manual, printed clinical practice guide that will be placed in a strategic place for consultation and badges that describe the function that corresponds to each of the members.

“Efecto marzo”: impacto de los nuevos médicos residentes en la salud de los pacientes

Al inicio de los ciclos educativos de los residentes, estos individuos proveen servicios de salud bajo una inadecuada supervisión como un proceso de aprendizaje en el que adquieren nuevas habilidades y conocimientos salud bajo la premisa de “aprender haciendo”, mas la falta inicial de estas expone a los pacientes a una baja calidad de atención que repercute en su salud lo cual no es ético. En consecuencia, la presente tesina evalúa los posibles efectos que tiene la masiva entrada de médicos residentes en México durante el mes marzo. Para lo cual emplea dos métodos con datos administrativos de las Secretaría de Salud. El primero es Diferencias en Diferencias, el cual tiene el objetivo de estimar el contrafactual de forma creíble suponiendo que las características de los individuos son invariantes en el tiempo y que en ausencia de la intervención el grupo de tratamiento seguirá la misma tendencia que el grupo de control. El segundo método es un event study cuyas virtudes son aportar evidencia de una buena identificación del método DID y descomponer el efecto en ventanas de tiempo para analizar como los cambios en el tiempo de la inexperiencia, el burnout, las fallas de coordinación y el learing-by-doing afectan al análisis. Bajo estos diseños se encuentra que hay efectos negativos en la salud de los pacientes a causa de la rotación de residentes los cuales pueden ser explicados por los altos niveles de estrés y cansancio documentados, la falta de práctica real en los procedimientos médicos, la baja supervisión de los residentes y el paradigma educativo de “aprender haciendo”. Además, se observa que estos efectos negativos solo ocurren en las primeras semanas después de la entrada de residentes para después desvanecer su efecto en los meses siguientes, lo cual indica que los residentes logran superar las limitaciones y sobrecarga del trabajo para finalmente otorgar un servicio de salud correspondiente a otros meses.

La pérdida de audición se refiere a la disminución de la capacidad auditiva, éste es un proceso progresivo e irreversible, en el que la mayoría de las ocasiones el individuo que presenta pérdida no se da cuenta de su evolución hasta que el proceso se encuentra avanzado. Dado que la audición es la vía principal a través de la cual se desarrolla el lenguaje y el habla, se debe tener presente que cualquier trastorno en la percepción auditiva de un individuo en edades tempranas, afectará su desarrollo lingüístico y comunicativo, sus procesos cognitivos y consecuentemente su posterior integración escolar, social y laboral. Todas estas consideraciones explican la necesidad de detectar este problema en sus comienzos, incluso antes de que el individuo se percate de que existe. La audiometría es una exploración del sistema auditivo, en la cual se mide la capacidad del oído humano de percibir sonidos. El objetivo principal de esta tesis es el diseño y desarrollo de un audiómetro automatizado, que con el reconocimiento de imágenes pueda interpretar la respuesta de la persona al realizar evaluación audiometría, Para ello, se desarrolló un sistema de visión computacional. Se presentan algunos beneficios del audiómetro digital, tales como la realización de pruebas de manera estandarizada para cada persona, la facilidad de almacenamiento de los resultados en formato digital, y la ampliación del acceso a las pruebas auditivas en lugares remotas.

Hearing loss refers to the decrease in auditory capacity, which is a progressive and irreversible process. In most cases, individuals with hearing loss are unaware of its progression until it has advanced. Since hearing is the main pathway through which language and speech development, it is essential to consider that any impairment in auditory perception during the early stages of life will affect linguistic and communicative development, cognitive processes, and subsequently, their educational, social, and occupational integration. All these considerations highlight the need to detect this problem early on, even before the individual becomes aware of its existence. Audiometry examines the auditory system that measures the human ear’s ability to perceive sounds. The main objective of this thesis is the design and development of an automated audiometer that, through image recognition, can interpret a person’s response during audiological evaluations. To achieve this, a computer vision system was developed. Some benefits of the digital audiometer are highlighted, such as conducting standardized tests for each individual, ease of storing results in digital format, and expanding access to auditory tests in remote locations.
